    WCU Climbing Wall Guidelines, Informed Consent Statement

    There are certain medical conditions that may increase your risk to safely participate in the activity of rock climbing including but not limited to: seizures, arthritis, partial to full hearing or vision loss, recent soft tissue injuries, and high blood pressure. Keep in mind that conditions such as these may directly affect the safety of other participants. Please consult with a physician before becoming involved in this activity if you are unsure about your ability to participate.

    All climbers must have this form completely filled out or on file before climbing. Waivers are valid until the beginning of each Fall Semester. Each Fall Semester, each participant will have to re-sign the waiver.

    A legal parent or guardian must sign the WCU Climbing Wall Informed Consent Statement before anyone under the age of 18yrs old will be allowed to climb. Children under 14 years old must be accompanied by an adult.

    Read the Climbing Wall Rules as well as all posted signs and warnings. Adhere to the instruction and recommendation of supervisory staff.

    All climbing instruction will be done by WCU Climbing Wall Staff. Children must be 18yrs old to receive a belay lesson.

    All climbers are responsible for proper fit of their equipment, properly tying and dressing their knot, and double checking their belayer's setup. All knots must be tied directly to your harness. DOUBLE CHECK YOUR KNOT!

    All belayers are responsible for proper fit of their equipment, proper setup of their belay system, correct belay technique, and double checking their climbers setup.

    Belayers will be given a warning the first time they are caught belaying incorrectly including lowering the climber at the end of their climb. A subsequent occurrence will require the belayer to retake the basic lesson at the established price before being permitted to belay at the WCU Climbing Center in the future.

    Proper climbing commands will be used both before climbing and while on the wall. The purpose of these commands, to double check equipment and setup, are imperative to safe climbing.

    Bouldering is allowed for those who understand and demonstrate to the satisfaction of the Climbing Wall staff as understanding of the rules. When bouldering your head should not surpass taped line at 12 feet. We recommend using a spotter. Crash pads are provided. Bouldering under any climbers is prohibited.

    I agree that as to any equipment, which I provide or borrow or rent from WCU's Campus Recreation Climbing Wall during any climbing/or other indoor or outdoor activity, I use at my own risk. I understand and agree that WCU's Campus Recreation Climbing Wall shall not be liable for any loss, damage, or injury resulting from the use of said equipment.

    Non College students are required to wear a helmet at all times while on the wall and on the carpeted area.

    Climbing under the influence of an intoxicating substance will not be permitted.

    Climbing Wall staff has the right to suspend privileges to this facility without notice, or refund for any action/activities deemed inappropriate or unsafe.

    I hereby acknowledge that I have been informed of the nature of the activity and assume the inherent risks involved in indoor rock climbing. I am aware of the hazards and risks which may be associated with my participation in the above-named activity, including the risk of bodily injury or possibly death resulting from my personal health/physical limitations known and unknown, malfunction or misuse of equipment, failure to adhere to proper climbing procedures, failure to acknowledge personal skill levels, decision making, or accidents. I understand, accept, and assume all such hazards and risks, and waive all claims against the University, their officers, agents, employees, and any other persons or entities acting on their behalf. I understand that I am solely responsible for any costs arising out of any bodily injury or property damage sustained through my participation in normal or unusual acts associated with the above-named activities.

    The West Chester University Climbing Wall staff is comprised of West Chester University student employees trained in rock climbing, risk management, and CPR/First Aid. They are educated in the implementation of the policy/procedures manual established for the facility.

    I understand that I may not use the WCU Climbing Wall until I have been deemed qualified to do so by the Climbing Wall staff. I certify that I am a legally competent adult 18 years of age or older and have read this paragraph releasing West Chester University, its' agents, officers, employees, successors, volunteers, and representatives from liability, and I understand and voluntarily sign this form.

    I, the undersigned participant, exercising my own free choice to participate voluntarily in the above-named activity, and promising to take due care during such participation, hereby release and discharge, indemnify and hold harmless the University, PASSHE, the State System of Higher Education, its Trustees, officers, agents and employees of the sport club program, from and against any and all claims, liability, losses, third-party claims, damages, costs, or expenses (including attorneys' fees), from any responsibility or liability in case of personal injury, equity, disability, or death sustained by me or damage to property of others caused by me during or because of participation in the activity of the Climbing Wall program.

    I am aware of the inherent risk involved in the sport of rock climbing. I understand that the WCU Climbing Wall makes helmets available to all participants at no cost. I am aware that wearing a helmet may reduce the risk of serious injury or death. With this understanding I may choose not to use this important safety device.
